Pharmacokinetics and Indications: Tadalafil citrate is a popular drug used mainly for erectile dysfunction. Tadalafil is a PDE5 inhibitor. Erection is the result of pooling of blood in penile vessels. Tadalafil increases the supply of cGMP in blood vessels by inhibiting PDE which dilates penile blood capillaries and smooth muscle of corpus cavernosum. Tadalafil has a longer half life i.e. about 19 hours; this property has earned this drug the name of WEEKEND PILL. Its action can last up to 36 hours. It only exerts its effect if there is sexual stimulation, as nitric oxide is only released on sexual stimulation. The main indication of Tadalafil is treatment of erectile dysfunctions due to many reasons for example diabetes mellitus and psychosomatic causes of impotence. Tadalafil has also been approved by FDA for the treatment of pulmonary hypertension. Tadalafil is available in form of 5mg, 10mg and 20mg tablets. The recommended starting dose for erectile dysfunction is 10mg OD. This drug should not be used more than once a day. The recommended dose for treatment of pulmonary arterial hypertension is 40 mg once daily.
Possible Side effects: Most common side effects are headache, back pain, muscle pains, nasal congestion and runny nose. Most of these side effects disappear after 48 hours of last intake. As tadalafil exerts its effects by relaxing smooth muscles in blood vessels, there is potential of low blood pressure. Patients who are already on nitrates are advised not to take nitrates at least for 48 hours of last dose of tadalafil. Patients of angina using tadalafil are advised not to use their spray in the event of chest pain but to contact their physician. Other documented side effects are visual and hearing impairment.
Effective dose: 10 – 40 mg per day
Its effcet lasts up to 36 hours. Should be taken accordingly and on empty stomach.
